Team Entry Fee
The entry fee of $160 per team includes the Big Bass Open Tournament for all Anglers. By completing the above form and submitting this payment you confirm that I have read and understood the description of the tournament and the official rules of the tournament. I will obey all rules and understand that any violation may result in immediate disqualification without a refund of the entry fee. I also understand the tournament officials may reject my application for any reason and in that will refund my entry fee. I release the Jake Wilson Memorial Foundation and JC Outdoors from all Liability and give permission to use my name and photograph for promotional purposes. I agree to submit to and accept the results of the Polygraph test given at this event. Your above application reflects that i have read the entire documents averrable for download below and understand it completely, understand that it and other documents I have signed as part of the process affect my legal rights, and agree to be bound by those terms for this event. Reference herein to "I", "myself", and other personal references shall include any child or ward for which I sign.
